この個人情報保護方針は、2023年10月1日の年次総会での承認を経て正式運用を開始いたしました。今後も運用にあたっては、会員の皆様一人一人の本方針への継続した賛同が重要です。引き続きご関心をいただき素直なご意見を頂ければうれしいです、どうぞご理解いただきながら、よろしくお願いいたします。また、もし私ども同窓会の個人情報の運用に疑問がございましたら、本HPの一番下に記載のあります問い合わせにあります宛先にご連絡いただけますようお願いいたします。またさらに皆様の個人情報の同窓会による利用を止めたい場合は、同様にご連絡をお願いいたします。頂きまいご連絡をもとに当該会員様の個人情報の運用を停止したします。どうぞよろしくお願い致します。

個人情報保護方針

はじめに

インディアナ大学同窓会日本支部（Indiana University Alumni Association Japan Chapter、以下、「IU日本同窓会」という）の行う活動目的は、

「インディアナ大学同窓生とその友人の親睦を深める手段を提供する。インディアナ大学に貢献しその繁栄を図る。」(会則第2条支部の目的)[日本語訳]です。

IU日本同窓会はこの目的実現のための事業を行い、その一環として会員および関係者の個人情報を活用いたします。

その上でIU日本同窓会が取得し取扱う会員および関係者の個人情報の保護が重要な責務であることを認識し、以下のとおり個人情報保護方針を定め、これを遵守いたします。

１．IU日本同窓会が取得する個人情報・個人関連情報について

１）IU日本同窓会が会員および会員登録をされる方から、取得する個人情報は以下のとおりです。

・氏名

・連絡先（電話番号及びメールアドレス、又は両方）

・住所

・入学（所属開始）年度

・卒業（修了）年度

・所属学部

・取得学位（又は資格）。

次の情報についてはご本人の判断により任意の提供となります。

勤務先または在学校名、所属先の肩書

それに加え、個人関連情報（個人情報以外の個人の関心や興味・事情）を取得することがあります。

２）同窓会にご連絡頂く関係者から、氏名・連絡先（電話番号及びメールアドレス、又は両方）・住所等の個人情報や個人関連情報を取得することがあります。

３）会員や関係者のインタ―ネット上にある情報から個人情報・個人関連情報を収集することがあります。

４）ホームページ（HP）（ https://iuajapan.net/）上に搭載する会費・寄付納入決済システムSTRIPEをご利用いただいた場合は、そこに入力いただいたクレジットカード番号を除く、氏名、住所、メールアドレス、の個人情報をその管理上取得します。

２．個人情報・個人関連情報の利用目的

IU日本同窓会は取得した個人情報・個人関連情報を、会員の管理、会員への通知・案内および会報や同窓会名簿等の発行・送付、会員の状況分析、会員動向の把握、会員拡大のための諸活動に使い、また関係者支援の為の連絡等、本会の活動目的のみに使用します。

その際、必要に応じたインディアナ大学同窓会米国本部・他国支部への個人情報・個人関連情報の第三者提供が含まれます。

また、会報や案内等の印刷物の作成、会員への郵送に於いて印刷・郵送業者への個人情報の第三者提供を行います。

３．個人情報・個人関連情報の取得

IU日本同窓会が個人情報・個人関連情報を取得するのは以下の場合となります。

① IU日本同窓会がご本人との双方向のやり取り（電子メール、電話）を通じて取得する場合

② ご本人より、HP（https://iuajapan.net/) のオンラインフォームを通じてご提供いただく場合

③ ご本人より、一方的に直接事務局にご提供（葉書や手紙、郵便振替の通信欄）いただく場合

④ ご本人ではなく、ご本人以外の方（同窓生の会員、ご家族等）からIU日本同窓会にご提供いただく場合

⑤ ご本人が HPを通じて年会費・寄付金納付をするにあたり、その決済システムSTRIPEに必要事項を入力することに付随してIU日本同窓会に（クレジットカード情報以外の情報が）提供される場合

⑥ インディアナ大学同窓会米国本部・他国支部の第三者より提供される場合

⑦ IU日本同窓会が、インターネット情報を通じて、取得する場合

４．個人情報・個人関連情報取得における利用目的の通知・公表と本人同意

IU日本同窓会は以下の手段でご本人への利用目的の通知・公表を行います。

本「個人情報保護方針」をHP（https://iuajapan.net/）に掲載することで、利用目的を公表します。

IU日本同窓会は以下の場合において、ご本人の同意を得ます。

電話及び電子メール等の双方向通信手段を用いるやり取りの場合は、そのやり取りの中で本人同意を得ます。

HPのオンラインフォームを通じた個人情報提供の場合は、そのフォーム内に利用の目的を明示し、そこに都度本人同意を得ます。

会員より、葉書、手紙、郵便振替の通信欄等の郵便物による一方的な個人情報の提供がありえますが、会報等による事前の個人情報保護方針の通知を行います。

５. 個人情報の開示・訂正・利用停止について

会員が自己の個人情報について、開示、訂正及び利用停止を求める権利を有していることを確認し、これらの要求がある場合には、会員からの請求であることを確認の上、速やかに対応します。

６．個人情報の第三者への情報提供をする場合と提供を受ける場合について

6.1 個人情報の第三者への情報提供をする場合

IU日本同窓会では、会の目的に沿った事業を行うために、必要に応じてご本人の同意を得て、以下のような第三者への個人情報の提供を行います。

１）ご本人からの質問や他の方の質問に対応する目的で、本人同意の取れた会員および関係者の情報を、インディアナ大学同窓会米国本部あるいは他国支部に提供します。

2）業務を委託する外部業者に対して、以下のような場合に情報を提供します。

　-　HPを通じてご本人が年会費・寄付金納付をいただく場合に於いて、本人同意を経て、その決済システムSTRIPEに必要事項を入力いただくことで、氏名、住所、メールアドレス、クレジットカード番号を含む個人情報がSTRIPE社に提供されます。同時にそこで提供された個人情報のうち、クレジットカード番号を除く情報がIU日本同窓会にも提供されます。（STRIPE社での個人情報の取扱いに関しては、「8.クレジットカードによる年会費及び寄付金の支払いについて」をご覧ください）

– 会報や案内等の印刷物の作成,会員への郵送に於いて,印刷・郵送という作業の全部または一部を委託する形で外部業者を使います。ここではその外部業者に作業上必要な個人情報の提供を行います。

３）会員に、個人情報を含む会報や会員名簿をお送りする場合。

４）警察や裁判所等の公的機関から法律に基づく手続において照会を受けた場合、会員の行為によって同窓会規約等に反し、同窓会の権利や財産等を保護するため必要と認められる場合、及び人命・身体・財産等に対する緊急の必要性がある場合。

6.2 個人情報を第三者から提供を受ける場合について

IU日本同窓会では、ご本人の同意を既に得ている個人情報の提供を、以下のような場合に第三者から受けることがあります。

１）本人同意の取れた会員または関係者の情報を、インディアナ大学同窓会米国本部あるいは他国支部より提供を受け、ご本人または他の方からの質問に対応する場合

７．個人情報の管理について

IU日本同窓会では、個人情報を正確かつ最新の状態に保ち、不正アクセス・紛失・破壊・改ざん・漏洩、または再提供などの無いように、適切な管理を実施します。

また、個人情報を第三者に提供を行う際には、その記録を作成・保存し管理いたます。さらに、第三者から個人情報の提供を受ける場合も同様に、第三者における情報取得時の本人同意を確認し、その扱いを他の個人情報と同様に管理いたします。

８．クレジットカードによる同窓会費及び寄付金の支払いについて

HPを通じて年会費・寄付金納付をいただく為に、外部委託業者STRIPE社の決済サービスシステムを導入しています。IU日本同窓会としては、STRIPE社のプライバシーポリシーが本会及び会員に著しい不利益とならないと判断をし、この決済サービスシステムを活用しております。ただ一方で、STRIPE社のプライバシーポリシーは、本会の個人情報保護方針とは全く独立なものです。本システム利用の有無は、会員の判断に委ねるものです。

Introduction

The purpose of the Indiana University Alumni Association Japan Chapter (IUAA-JP) is to

“To provide a means for the association of Indiana University Alumni and friends. To advance the interests and promote the welfare of Indiana University.”　(Constitution, Article II – Purposes ) 　

The IUAA-JP will conduct activities to realize this purpose and will use the personal information of its members and related persons as a part of such activities.

The IUAA-JP recognizes that the protection of the personal information of its members and related parties obtained and handled by the IUAA-JP is an important responsibility and has established and complies with the following privacy policy.

1.Personal Information and Personally Identifiable Information Obtained by the IUAA-JP

(1) The IUAA-JP collects the following personal information from its members and those who register as members; Name, contact information (telephone number and/or e-mail address), address, year of admission (start of affiliation), year of graduation (completion), faculty affiliation, and degree (or credential) obtained.

The following information is provided voluntarily at the discretion of the individual.

Name of employer or current school, title of affiliation.

In addition to this, we may obtain personal related information (personal interests, interests, and circumstances other than personal information).

(2) We may obtain personal information such as name, contact information (telephone number and/or e-mail address), address, and other personally identifiable information and personally relevant information from persons who contact the Alumni Association.

(3) Personal and personally-related information may be collected from members and related parties through information available on the Internet.

(4) If you use the STRIPE membership fee and donation settlement system on the homepage (https://iuajapan.net/), we will collect your name, address, e-mail address, and other personal information, excluding credit card numbers entered there, for the purpose of managing the system.

2. Purpose of Use of Personal Information and Personally Identifiable Information

The IUAA-JP will use the personal information it collects for the sole purpose of managing members, issuing and sending notices and information to members, publishing and sending newsletters and alumni lists, analyzing membership status, monitoring membership trends, conducting various activities to expand membership, and communicating with related parties for the purpose of supporting the activities of the Association.

This includes the provision of personal information and personal-related information to the U.S. headquarters of the Indiana University Alumni Association and its branches in other countries, as necessary, to third parties.

In addition, personal information will be provided to third parties that are printing and mailing companies for printing materials such as newsletters and announcements, and for mailing to members.

3. Acquisition of personal information and personally-related information

The IUAA-JP collects personal information and personally identifiable information in the following cases

(i) When IU Alumni Association Japan obtains personal information through interactive communication (e-mail, telephone) with the person in question.

(ii) When the person provides personal information through an online form on the IU Alumni Association’s homepage (https://iuajapan.net/)

(iii) When the person provides the information directly to the Alumni Association through a postcard, letter, or postal transfer form.

(iv) When the information is provided to the IUAA-JP by a person other than the person himself/herself (e.g., alumni member, family member, etc.)

(v) When you provide information (other than credit card information) to the IUAA-JP in connection with entering the required information into the STRIPE payment system when paying your annual dues or donations via the IUAA-JP’s homepage.

(vi) When information is provided by a third party of the Indiana University Alumni Association, U.S.A. Headquarters or other branches of the Indiana University Alumni Association

(vii) When the IUAA-JP obtains the information through Internet information.

4. Notification and publication of the purpose of use of personal information and personally identifiable information and consent of the individual to the use of such information

IUAA-JP will notify and publicize the purpose of the use of personal information by the following means.

By posting this “Privacy Policy” on its homepage (https://iuajapan.net/), the IU Alumni Association will publicly announce the purpose of use.

IUAA-JP will obtain your consent in the following cases

In the case of communication using two-way communication methods such as telephone and e-mail, we will obtain the consent of the person during the communication.

When personal information is provided through the online form on the homepage, the purpose of use is clearly stated in the form, and consent is obtained in each case.

Members may unilaterally provide personal information by postcards, letters, postal money orders, and other mailings, but we will notify them of our privacy policy in advance through newsletters and other means.

5. Disclosure, Correction, and Suspension of Use of Personal Information

We will confirm that members have the right to request disclosure, correction, and suspension of use of their personal information, and if such a request is made by a member, we will promptly respond to the request after confirming that it is from the member.

6. When to Provide Personal Information to Third Parties and when to receive such Information

IUAA-JP will provide personal information to third parties as follows, with your consent as necessary, in order to conduct activities in accordance with the objectives of the Association.

(1) Information on members and related parties with your consent will be provided to the U.S. headquarters of the Indiana University Alumni Association or its branches in other countries for the purpose of responding to your questions or those of others.

(2) Information will be provided to outside contractors to whom we outsource our services in the following cases

　- When you pay your annual membership fee or donation through our homepage, your personal information including your name, address, e-mail address, and credit card number will be provided to STRIPE when you enter the necessary information into the STRIPE payment system after you give your consent to do so. At the same time, the personal information provided to STRIPE, excluding the credit card number, will also be provided to the IUAA-JP. (For more information on STRIPE’s handling of personal information, please refer to “8. Payment of Annual Dues and Donations by Credit Card”)

– IUAA-JP uses outside contractors to handle all or part of the printing and mailing of newsletters, brochures, and other printed materials to its members. We will provide personal information necessary for the work to the outside contractor.

(3) When we send newsletters or membership lists containing personal information to our members.

(4) In the event that the IU Alumni Association receives an inquiry from the police, a court, or any other public agency in accordance with the law, or if a member’s actions violate the Alumni Association’s rules and regulations and it is deemed necessary to protect the rights or property of the Association, or if there is an urgent need to protect human life, limb, or property.

IUAA-JP may receive personal information from a third party, for which the individual’s consent has already been obtained, in the following cases

(1) When we receive information about a member or a related person for whom we have obtained his/her consent from the U.S. headquarters of the Indiana University Alumni Association or its branches in other countries to respond to questions from the person or other persons.

7. Management of Personal Information

The IUAA-JP will keep personal information accurate and up-to-date, and will implement appropriate management to prevent unauthorized access, loss, destruction, alteration, leakage, or re-distribution.

In addition, when personal information is provided to a third party, a record of such provision is prepared, stored, and managed. Furthermore, when receiving personal information from a third party, we will confirm the consent of the individual at the time the information is acquired by the third party and manage it in the same manner as other personal information.

8. Payment of Alumni Dues and Donations by Credit Card

IU Japan Alumni Association uses the payment service system of STRIPE, an outsourcing company, for payment of annual membership dues and donations through the homepage. We believe that STRIPE’s privacy policy is not significantly detrimental to the IUAA-JP and its members. However, STRIPE’s privacy policy is completely independent from the privacy policy of our association. The decision to use this system is left to the discretion of the member.

The IUAA-JP will manage and use personal information and personal-related information in accordance with the laws and regulations of Japan. The IUAA-JP may revise the Policy on the Protection of Personal Information to comply with changes in laws and regulations, to ensure the protection of personal information, or for other reasons.
